GENERAL INFORMATION

Confidentiality

You can be assured that all personal communication shared with us will be kept in strict confidence. We are sensitive to the needs of developing a trusting relationship in a safe setting so that you will feel comfortable expressing your thoughts and feelings. Whenever it becomes necessary to disclose information, as in the case of insurance benefits, your consent will be requested beforehand.

Telephone Response

When we are in session or away from the office, our messages are recorded by voice mail 24 hours a day. Typically, we return calls the same day they are made. While we make every effort to respond promptly, our schedules occasionally delay our responses by several hours. Sometimes, we may not be able to reach you until the following day. If an emergency should arise, you may follow the voice mail instructions to page us directly, and we will return your call as soon as we possibly can.

Availability

If we are out of town, we are sometimes available by phone in case of emergency. The PPC staff therapist who is on-call to receive emergency pages will return your call directly, then contact us if necessary. You may consider calling this on-call therapist to schedule an appointment if desired. Otherwise, in an urgent situation, we would encourage you to call a local 24-hour crisis line such as Crisis Care (937) 224-4646 or the Suicide Prevention Center (937) 229-7777.

Appointments and Cancellation Policy

To schedule a convenient time, please call us as early as possible for an appointment. We are available for appointments from 8:00 a.m. until 8:00 p.m. Monday through Thursday and until 5:00 p.m. on Friday, depending on your therapist’s schedule. Some therapists also offer Saturday appointments.

If you must cancel an appointment, please give us at least 24 hours notice. We have reserved this time especially for you. We will do everything possible to see you on time. Your promptness is appreciated as well.

Payments/Insurance Benefits

Most insurance policies include partial coverage for the services we provide. To keep your costs down and avoid excessive billing, we request payment at the time of service, unless prior arrangements have been made. We accept payment by Visa and MasterCard, and we have several payment plans to meet your needs.

While we will assist you with insurance forms for filing claims, we strongly urge you to call your insurance company regarding their rates of reimbursement for outpatient mental health services and your specific policy requirements for pre-certification of services. Also, we may be able to make special arrangements for some financial situations. Please feel free to discuss any questions you may have.

Situated in the south part of Dayton, we are located in a quaint, red brick house just north of Patterson Road on the corner of Wilmington and Colwick Avenues. The atmosphere is warm and comfortable in a setting not unlike a family room. Children are welcome to bring toys with them.

Directions:

From 675 Southbound:
Take 35 west to the Smithville exit (turn left). Turn right on Patterson Road, turn right on Wilmington and the office is shortly thereafter on the right side, on the same side of the street as RiteAid (next building up).

From 675 Northbound:
Take 675 north to the Wilmington Pike Exit (turn left). At the Wilmington/Smithville intersection Wilmington Pike splits off to the left. Stay on Wilmington. Cross Patterson Road and the office is shortly thereafter on the right side, on the same side of the street as RiteAid (next building up).

From 75:
Take 75 to 35 east to the Smithville exit (turn right). Turn right on Patterson Road, turn right on Wilmington and the office is shortly thereafter on the right side, on the same side of the street as RiteAid (next building up).
